Salary Range: $41.53 - $46.35 per hour

Work Hours: 40 hours per week, Monday - Friday, with no weekend rotation

Employment Type: Full-time

Location: 84 bed Skilled Nursing Facility

Job Summary:

We are seeking a highly skilled and compassionate Physical Therapist to join our team. As a Physical Therapist at our facility, you will work with a variety of patients, including those recovering from injuries, surgeries, or illnesses, as well as those with chronic conditions. You will be responsible for developing and implementing individualized treatment plans to help our patients improve their mobility, reduce pain, and achieve their functional goals.

Key Responsibilities:

- Evaluate patients' physical abilities, limitations, and medical history to determine the most effective treatment plan

- Develop and implement individualized treatment plans that include therapeutic exercises, manual therapy, and other modalities

- Monitor and document patients' progress and adjust treatment plans as needed

- Educate patients and their families on exercises and activities to promote recovery and prevent future injuries

- Collaborate with other healthcare professionals, such as doctors and nurses, to ensure comprehensive and coordinated care for patients

- Maintain accurate and up-to-date patient records and documentation

- Adhere to all legal and ethical standards and guidelines in the provision of physical therapy services

Qualifications:

- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program

- Current state license to practice as a Physical Therapist

- Minimum of 1-2 years of experience in a skilled nursing facility or similar setting preferred

- Strong knowledge of physical therapy principles, techniques, and modalities

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a multidisciplinary team

- Compassionate and patient-centered approach to care

- Basic computer skills and proficiency in electronic medical records
