Job Title: Physical Therapist

Salary Range: $49-$55 per hour

Job Type: Full-time, Monday-Friday, no weekend rotation

Work Setting: Skilled Nursing Facility (SNF)

Job Summary:

We are seeking a skilled and compassionate Physical Therapist to join our team at our Skilled Nursing Facility. As a P-Physical Therapist, you will be responsible for providing physical therapy services to patients in our facility. You will work closely with our interdisciplinary team to develop individualized treatment plans and help patients achieve their maximum potential for physical function and independence.

Key Responsibilities:

- Conduct initial evaluations and assessments of patients' physical abilities and limitations

- Develop individualized treatment plans based on patients' needs and goals

- Implement and monitor treatment plans, making adjustments as needed

- Provide hands-on therapy, exercises, and other interventions to improve patients' strength, mobility, and range of motion

- Educate patients and their families on proper techniques for exercises and activities to improve their condition

- Document patient progress and communicate with other members of the interdisciplinary team

- Collaborate with physicians and other healthcare professionals to ensure comprehensive care for patients

- Maintain accurate and up-to-date patient records

- Stay current with advancements and best practices in physical therapy through continuing education and professional development opportunities

Qualifications:

- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program

- Current state license to practice as a Physical Therapist

- Minimum of 2 years of experience in a SNF setting preferred

- Strong knowledge of physical therapy principles and techniques

- Excellent interpersonal and communication skills

- Ability to work independently and as part of a team

- Compassionate and patient-centered approach to care

- Basic computer skills and experience with electronic medical records systems
