Salary Range: $41-45 per hour

Position Type: Full-Time, Monday-Friday with weekend rotation

Facility: SNF Community offering post-hospital, short-term, long-term, and respite care

Number of Beds: 181 SNF beds

Job Description:

We are seeking a highly skilled and compassionate Physical Therapist to join our team at our SNF community. As a Physical Therapist, you will work with a diverse patient population, providing rehabilitation services to individuals with a variety of conditions and injuries.

Responsibilities:

- Conduct initial evaluations and assessments to determine patients' needs and develop individualized treatment plans

- Provide hands-on therapy and exercises to improve patients' strength, mobility, and range of motion

- Educate patients and their families on exercises and techniques to continue their rehabilitation at home

- Monitor and document patients' progress and adjust treatment plans as needed

- Collaborate with other healthcare professionals, including doctors and nurses, to ensure comprehensive care for patients

- Communicate effectively with patients, families, and caregivers to provide support and guidance throughout the rehabilitation process

- Adhere to all legal and ethical standards in patient care and documentation

- Participate in continuing education and professional development opportunities to stay current with best practices and advancements in the field of physical therapy

Requirements:

- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program

- Current state license to practice as a Physical Therapist

- Minimum of 1-2 years of experience in a similar setting preferred

- Strong communication and interpersonal skills

- Ability to work independently and as part of a team

- Compassionate and patient-centered approach to care

- Knowledge of and adherence to all legal and ethical standards in patient care
